instructions for Schedule Chart? format of input dataset - example requested!!

Just starting out using the Schedule Chart object. Does anyone have a straightforward small example dataset they can share? looking for an example for formatting.... or perhaps simply reply back with "column" names/types ?? Thanks in advance!!
