I have a series of earlier-produced RTFs (legacy outputs) and I need to make a TOC. Concatenating the RTFs is no issue, but how can I make the TOC without re-running the code to produce them? None of the prior produced RTFs used PROCLABEL during production.

If you have consistent styling on your headers, look into Words autodetect headings option and can build the TOC using Word.
This process can then be automated via PowerShell or VBA depending on exactly how automated of a process you need.
