Sorry for reviving an old post, but I just encountered this issue and found a fix. For me it was the Windows OS Display Setting, 'Scale and Layout'. When set to 150% (possibly above too) it causes this. Another area affected is in the prompt manager, when you edit the static list, the default column does not show the checkboxes.   Changing the 'Scale and Layout' setting to 125% or 100% fixed this for me.
