At this point I would say it is time to ask "What are you going to do with this data?". It may be that you have a thought for analysis that restructuring the data might be a better than this many variables per record. I have found that almost every time analysis involves dates I do not want to work with some character date substitute and it may be that having data in the form of : PatientID Date Medicated 1 1/1/2000 1 1 1/2/2000 0 1 1/3/2000 0 1 1/4/2000 0 1 1/5/2000 1 etc. leads to easier analysis.
