I see that you can use the CONCORDANCE option with PROC PHREG to compute Harrell's Concordance Statistic, but I can't seem to figure out a way to have 95% confidence limits included in the output. Would I have to use CONCORDANCE=HARRELL(SE), and then calculate the 95% CI as Estimate +/- 1.96*(Standard Error) ?

Yes. Since the delta method is used to obtain the standard error, it is assumed that the statistic is asymptotically normal.
