(1-PH) Statistics or Proportional Hazards

I would like to perform (1-PH) Statistic or Proportional Hazards to validate the models.

I got actual and predicted values. Is there any PROC or something which can help me to achieve this.

Moreover, it would be nice if there is a reference formula.
