I want to get a table like the this:
How can I put the top column ("Recruitment" and "Post-Recruitment") in the report? I used proc report to make it but did not succeed.

Do you mean the top ROW?
Something like this in PROC REPORT
columns group ("Recruitment" declined currently consented)
("Post-Recruitment" Completed partially Pending);
where variables are assumed to be named DECLINED CURRENTLY CONSENTED etc.
