Special Customization - Printing Report

I have attached sample "comments2.sas7bdat" sas dataset and Desired_output_Comments2.xls.

I would like to print the report as shown in the Desired_output_Comments2.xls file. Any suggestions or other ideas?

You're a lot more likely to get an answer if you

1. Provide your data as code so we can easily use that data

2. Provide your desired output as an image. Many people will not download excel files

3. Ideally, simplify the data and the report to focus on the issue you face, rather than providing too much data

4. Ideally, show some code you've tried ad explained what issues you face
