How to set outcome distribution to binary in PROC CAUSALMED?

The documentation (SAS/STAT 14.3) says that there's a DIST=BIN option of the MODEL statement for this purpose and also that "binary" is the default if the outcome variable is listed in the CLASS statement. (I have never used this procedure.)
