keep sequence in proc transpose

Hi, I'm trying to keep the date of a group of accts. and transpose it into the column name along wiht the score.. here's what I mean..

Have

Acct    Date     Score

001     Jul12     234

001     Aug12   235

001     Sep12   364

002     Jul12    535

002    Aug12   534

002    Sep12    535

Want

Acct   Jul12_score     Aug12_score         Sep12_score

001        234                    235                         364

002         535                    534                         535
