Solved: How to request OR and CL(lower and upper) in PROC LOGISTIC?

Hello:

I have the following PROC LOGISTIC procedure. The result shows the analyst of Maximum Likelihood Estimate with Wald Chi-Square result. I am also interested in obtaining the Odds Ratio and CL. How to add them? Please advise, thanks.
