Picking matched controls

I need to match cases and controls on 5 different criteria. 4 of these are demographics and the 5th is a comorbidity score which I can calculate only after I have narrowed my control pool down using the 4 demographic criteria. My dataset of cases is ready. What is the easiest method to pick matched controls ?
