To have converged, should the 3 criteria reported on page 3.7 of the course text be all met at the same time or would it be enough for one of them to be met to declare converge of the algorithm?

Just one. The number of "times" the criteria needs to be satisfied in order to be deemed as converged can be changed in the optimization properties window. The default is one, for each of the specified criteria.
