After data manipulation and calculation, I got the data set (see the top of the attachment). I want the report with the format (See the bottom of the attachment). Any idea or suggestions will be greatly appreciated!

Many users here don't want to download Excel files because of virus potential, others have such things blocked by security software. Also if you give us Excel we have to create a SAS data set and due to the non-existent constraints on Excel data cells the result we end up with may not have variables of the same type (numeric or character) and even values.

 

For a SAS data set to describe what you have best is to post data step code to replicate your data (or part). Instructions here: https://communities.sas.com/t5/SAS-Communities-Library/How-to-create-a-data-step-version-of-your-dat... will show how to generate data step code from a SAS data set that can be posted here using the code box icon {i} in the message menu or attached as a TXT file.
