Safety table from table to report using PROC REPORT

I have the dataset that is the base for Safety report. Could you please help to write the Proc Report to issue the correspondent report.

HI:

  Please review the paper and look at Example 1 and Example 8 -- there are actually 2 different versions of Example 1 in the paper. Both of those were done with PROC REPORT and Example 8 was done with Table Template and DATA _NULL_ step.
