I am in search of the data underlying the 3-level model (mctrial dataset, page 6). I would like to play around with several covariance structures and also use it as an example in our courses on quantitative veterinary epidemiology (where we exclusively use SAS). Unfortunately the email address of the author of this paper does not exist anymore. Hope somebody can help!

There is a data step on the same page which creates the data, albeit with only a partial DATALINES block. Fill the DATALINES with additional fake data.
Thanks Kurt, it will not be that easy to simulate a 'nice' distribution over the levels as only 11 out of 288 records are printed. I think the data originate from:

Any other 3-level dataset is welcome as well as of course!
