Convert a measure to category

Hi - looking to convert an 'id' integer which has been taken in as a measure but wish to make it a category. Can anyone let me know what the empty fields in the image should be entered with. Thanks.

You will need to define the name of the format or informat along with its definition.  See SAS Help Center: Concepts PROC FORMAT
