Do you have your PROC PHREG code already?
I think you can ask for this plot by one-or-another option.
I suspect your time and age variables need to be numeric as well (date, time and date-time variables in SAS are also numeric).
You might also want to look into PROC PROBIT :
It is mainly designed for dose-response analysis with a natural response rate. It computes the fiducial limits for the dose variable and provides various graphical displays for the analysis.
