I am looking for sample SAS Sizing Questionnaire being filled out. Does anybody has any relevant documentation, or know where I can get it from ?

Talk to your SAS representative. This is normally a thing done by SAS.

I didn't want to be the first giving such an answer - but yes, totally agree with Linus. Sizing is a specialized task done centrally in Cary by SAS for all countries.

On a side note: Please post your questions in one forum only. That's good enough for us to see. I'd suggest you delete all your identical posts which haven't got a reply yet.
