At this point, since I cannot duplicate your issue, if your code is essentially the same as my test code, I have no idea what might be wrong. This is the point at which you need to start working with Tech Support.

If you open a track with Tech Support, they can look at all your code and at your output PDF and they can help you resolve this issue.
