I am studying a biomarker for predicting the blood pressure response and I want to determine the best cutoff for this biomarker using Youden's index. How can I do that in SAS? and what is the code that I can use to determine that?

i'm not familiar with youden's index but i guess it is not your only option. Eg these people at the mayo clinic make a couple of SAS macros available for the purpose ie %cutpoint and %findcut: http://www.mayo.edu/research/documents/biostat-79pdf/doc-10027230
