Hello,

 

I was wondering if anyone could share any world-leading papers and knowledge relating to visionary thinking and empirical results around proactive individualised health care especially under the lens of patients with specific needs, conditions or disabilities to be able to identify patients who are potentially in danger of losing the support structures they have around them.  Any links to SAS statistical approaches, models, methods, code or procedures would be greatly appreciated as well.
