Thanks for your reply! Yes it would have the two consecutive underscores. What is odd is that the very same code works fine with my old dataset (the same as now just without some corrections). I've also tried excluding the ref_no variable totally from the procedure but it makes no difference.

I've got the same error, It was because the csv file was open in another application (excel), I closed excel and it worked fine
