Please advise how can i create the data which convert the below in 4 different columns

Please follow some general guidance on posting a quesiton:

Provide test data in the form of a datastep - i.e. something we can run and have the data.  You can see plenty of examples on other questions or here:

Next post what the output should look like, I cannot guess what it is supposed to look like?

 

Use descriptive thread titles.

is that currently a SAS data set? What are the names of the variables? What are the variable types? What names do you want for the output set. And WHAT does the output look like? There are several ways your request could be interpreted and some of them could be very poor ideas.
