Is there a simple way to center justify the values in a table generated using PROC FREQ?
Also, is there an option that will replace the variable name w/ the label? Right now, the lable is listed after the variable name in parantheses (e.g. diag(Type of Injury)).

Since SAS 9.2 the PROC FREQ crosstabs table has had its own table template that allows considerable customization. See this SAS Note regarding your 2nd question. While you're modifying the CrossTabFreqs template you can also change it to center justify the values.
As soon as you start to want to customize PROC FREQ you should look into PROC TABULATE instead, which allows style attributes and more control over labels/names.
Or in additional to @Reeza's comment, send the output of Proc Freq to a data set (option noprint and Output statement) and then use proc print or other proc that allows different controls to generate the desired appearance.
