how to write group information above the report body

I am having trouble at writing the group info above the report body using proc report.

This is what I want to do:
_page_
Name: John Doe
Age: 20
Address1: 123 Main St, City: Any, St: Any
-------------------------------------------------------------
var1 var2 var3 var4 var5
var1 var2 var3 var4 var5
Total for this client:

Start a new page with a new patient.

Any suggestions are appreciated.

Explore using PROC REPORT and also the BREAK statement, as well as other required statements too.
