assign sql database as a library in SAS EG

Hi Team ,

I am trying to assign the below database on a SQL server as a library in sas. I need to pass in user name and password as well..

Server : MELMD0226

database : Boss_Logging

Table : dbo.sales

Could you please advice how this can be achived ..
