Are you familiar with CDC's HCUP NIS database? That website has macro's for charlson comorbiditity index I think so (or Elixhauser's comorbidity index).

Hi Jack. There are a number of variations on the Charlson and a lot of different code sources.

SEER publishes a version of the code but it's specific to cancer populations:

https://healthcaredelivery.cancer.gov/seermedicare/considerations/macro-2014.html

There's a Canadian group that has a lot of fantastic resources. I have heard that Canadian dx patterns and codes could differ slightly from code patterns in the US, though, so that's a caveat.

http://mchp-appserv.cpe.umanitoba.ca/viewConcept.php?conceptID=1098

http://mchp-appserv.cpe.umanitoba.ca/Upload/SAS/_CharlsonICD10.sas.txt

There are also versions specific to Medicare data:

https://www.lexjansen.com/wuss/2013/119_Paper.pdf

Lastly, if you work for an organization that is part of HCSRN, there are standardized versions of the Charlson and other comorbidity indices available.

Hope this helps!
