How to design input prompt in table format using stored process facility?

I want to get the input from user in table format . for example , in below table - Date will be constant from 01/19 to 12/19, all other variable values need to get from user side and that tooo, user should give the value directly to the table instead of having multiple input fields. please guide me.

This could be done with jsp, but not wit the prompt facility.

Ok. Then is it possible to get multiple value for single varialbe. eg :Let say input prompt 'Enter Name' , user has to give list of name which should be stored in single variable. How to acheive that??
